SPINDLE/TUBE ATTACHMENT FAILURE.
Due to possible faulty welding the spindle carrying the wheel and hub assembly may detach from the axle tube.
The axle manufacturer is carrying out ultrasonic and magnetic checks on all potentially affected axles at the premises of all identified end users. The complete axle will be replaced in all cases where the weld is found to be suspect.

THROTTLE MECHANISM MAY NOT RETURN TO IDLE POSITION.
Under extreme climatic conditions eg low ambient temperature combined with high air humidity ice crystals may form in the crankcase ventilation system which includes the throttle housing. Should this condition occur ice build up may prevent the throttle from fully closing.
Recall the affected vehicles for the fitment of a modified throttle housing and a new vent hose.
CRACKING OF FLOOR ADJACENT TO HANDBRAKE MOUNTING POINTS.
Minor cracking of the floor can occur around the handbrake lever mounting points. If this is ignored the effectiveness of the parking brake may be reduced.
Recall the subject vehicles and fit reinforcing plates to the affected floor area.
SECURITY OF TRACK ROD ENDS
The torque of the track rod end retaining nuts may relax and allow the nuts to loosen.
Recall the affected vehicles and replace the existing track rod end nuts with quality assured examples.
FRONT SEAT SECURITY.
The front seats may not lock properly on the longitudinal adjustment/mounting rails.
Recall the affected vehicles for the fitment of new trigger springs to the seat adjusting mechanism.
ROAD WHEEL SECURITY
The wheel nuts do not meet the manufacturer's specification. This could result in road wheel loss.
Recall the affected vehicles for replacement of the original wheel nuts with quality assured examples.
BALL JOINT TO WISHBONE SECURITY
As a result of an assembly fault the bolts securing the ball joints on the wishbone may loosen. In extreme cases this could result in loss of steering control.
Recall the affected vehicles for a check on the torque of the ball joint mounting nuts/bolts. If any bolt is insufficiently torqued all three mounting bolts will be replaced.

FRONT BRAKE HOSES MAY HAVE BEEN INCORRECTLY INSTALLED DURING ASSEMBLY
It is possible that installation of the front brake hoses has not been correctly carried out during initial assembly.
Recall the affected vehicles for the fitment of replacement brake hoses.
DEFECTIVE BRAKE LIGHT SWITCH
The brake light switch may stick in the on or off position.
Recall the affected vehicles check and where necessary replace the brake light switch with a quality assured unit.
PROPELLOR SHAFT SECURITY
The gearbox output flange can come off the gearbox mainshaft whilst the vehicle is in motion and allow the propellor shaft to detach from the vehicle.
Recall the affected vehicles and replace the original flange with a modified version which allows full tightening of the flange retaining nut.
SERVICE BRAKE CONTROL VALVE
The service brake foot control valve may be defective. This could result in the loss of both braking circuits.
Recall the defective vehicles to check the subject valve. Defective valves will be replaced with a new unit.

REAR U BOLT SECURITY
The rear axle U bolts on some vehicles have been secured with galvanised nuts resulting in incorrect tightening torque and in some cases fracture of the U bolts.
Recall the affected vehicles for replacement of the galvanised nuts with phosphated examples.

The rear axle U bolts on some vehicles MAY have been secured with galvanised nuts as opposed to phosphated nuts. This has resulted in incorrect tightening torque and in some cases fracture of the U bolts.
Recall the affected vehicles and check the type of nut securing the rear axle U bolts. Where phosphated nuts are fitted - check torque setting and increase if necessary.
UNINTENTIONAL OPERATION OF THE MASTER SWITCH
The valve for pneumatic operation of the electrical main switch is so located that it is possible for the driver to operate the switch unintentionally. This could cause a total black out of the vehicle.
Recall the affected vehicles for replacement of the pneumatic valve with a modified unit.

POSSIBILITY OF INCORRECT ADAPTORS FITTED TO REAR BRAKE CALIPERS
There is a possibility that on some vehicles incorrect brake adaptors may have been fitted to the rear brake hydraulic calipers.
Optare Limited have contacted the operators of all the vehicles possibly affected and advised them to check the subject adaptors are of the correct type. Where incorrect adaptors are identified Optare will supply replacements of the correct
SEIZURE OF FRONT BRAKE ADJUSTER
The front brake caliper brake adjusting mechanism may seize due to the ingress of water over a period of time.
Recall the affected vehicles and replace the caliper with a version incorporating improved sealing arrangements.
FAILURE OF HEADLAMPS
A possibility exists that excessive torque generated when turning on the headlamp switch may induce an internal open circuit and cause the headlamps to go out.
Recall the affected vehicles for replacement of the headlamp switch with a revised component.
STEERING COLUMN FIXING BOLT OF INCORRECT SPECIFICATION
The fixing nut and bolt which secures the steering column to the steering box is not to the correct specification.
All subject vehicles to have the fixing nut and bolt replaced with quality assured items.
CRACKING OF DRIVING SEAT RAILS
The driving seat rail on manually adjustable seats may be subject to fatigue cracking.
